The Role:
An RN Case Manager at WVH is a passionate, empathetic, tenacious healthcare professional, willing to partner with patients and their families to get the most out of their remaining time together. This position is responsible for assessing, coordinating, and maintaining their assigned patient care plans and to meet with patients where they call home.
