Dementia Support

Dementia Support is a special program for people who have dementia and the person who helps care for them. It is paid for by Medicare and lasts for 8 years. This program is free and made to help you feel less stressed and have a better quality of life.

How to join the program

To join, you must:

  • Have a dementia diagnosis
  • Have Medicare Part A and Part B (not managed care)
  • Not live in a nursing home, assisted living, or foster care home

Our team visits your home every month. They give support, helpful tips, and connectyou with resources in your community. Some families may also qualify for short breaks(called respite care) during the year.

It doesn’t matter if you are newly diagnosed or have had dementia for a while—we are here to help. This is not a medical program. You will still call your doctor for medical care. But you can call us if you have questions or need help finding support.

When does the program start?

After your first visit, we send the paperwork to Medicare. It takes about 15–20
days to get approved. We will let you know when you are officially in the program.
