The Role:
An Admissions RN acts as a liaison between referral sources, patients/families, and Willamette Vital Health. Responsibilities include WVH program explanation to patients and families, as well as the initial nursing assessment and admission of patients to WVH programs.
We’re looking for RNs that want to use their expertise and experience as part of a team to provide the best care possible to those experiencing serious illness.

Requirements:
- Must be a registered nurse and graduate of an accredited school of nursing. I prefer a Bachelor of Science in nursing.
- Must have at least two years of clinical experience, part of which has been community based. Hospice experience preferred.
- Must possess a current license for nursing in the State of Oregon. Certification in hospice nursing or oncology nursing preferred.
- Must have a valid Oregon driver’s license, dependable independent transportation, and proof of current automobile insurance.
Essential Functions and Major Responsibilities:
- Demonstrate teamwork through cooperation, adaptability, flexibility, communication, dependability and support of team goals
- Demonstrates respect and care and maintains patient and staff confidentiality
- Effectively uses resources - time, people and supplies
- Demonstrates initiative, takes on projects willingly, acts on opportunities to improve and contributes new ideas
- Prepares and conducts admissions to WVH services
- Responsible for preparing referrals for appropriate response
- Establishes and documents hospice appropriateness through record review and/or telephone interviews with patient or caregivers
- Provides program explanation and obtains signed consents for hospice and palliative care referrals
- Make visits to admit patients and provide baseline data on patients
- Completes initial assessment for patient/family for patients
- Evaluates needs and assists with arranging for medications, equipment, supplies or transportation, and coordinates admission
- Acts as consultant nurse for any hospice patients in the hospital
- Works to constantly improve customer service to referral sources and patients/families
- Addresses, investigates, and works with manager to resolve any complaints related to referral/admission process
- Tracks and compiles requested data in a timely and accurate manner
- Documents according to established agency guidelines and professional practice standards
- Actively promotes WVH to referral sources with the goal of increasing referrals
- Supports the agency’s values, mission and goals and demonstrates this through the design and implementation of work activities
- Communicates the agency’s vision and actively involves staff in accomplishing agency vision, mission and goals
- Supports the work of the team to achieve long and short-term goals
- Creates a work environment that reflects a positive atmosphere, promotes employee satisfaction and competence, and displays strong evidence of teamwork that promotes quality outcomes and customer satisfaction
- Routinely integrates continuous quality improvement principles into practice
- May participate in a Performance Improvement Project (PIP) group, QAPI, and/or lead a PIP group
- Participates in or serves as facilitator of committees or meetings as assigned
- Communicates in an effective, accurate, and respectful manner
- Develops succinct, accurate and effective oral and written communications for staff, peers, the community and agency
- Plans, conducts and participates in staff meetings, in services and committee meetings as assigned
- Creates and maintains an environment where diversity is encouraged and honored
- Demonstrates competence in the area of responsibility and professional licensure
- Maintains high standards of professional practice by adhering to the ethics of the profession
- Maintains a knowledge base to ensure the provision of excellent care and to serve as a resource to staff
- Demonstrates continued professional growth and development through participating in education programs and review of current health literature
- Serves as informational resources for physicians, staff, referral sources and payer sources
- Performs other duties as assigned
Specific Job Skills:
- Working knowledge of and the ability to apply the basic principles and techniques of community health in the WVH program care settings.
- Must possess good clinical skills and be able to utilize problem-solving techniques.
- Knowledge of pain and symptom management.
- Strong interpersonal skills with expertise in customer service principles.
- Knowledge of hospice concepts and practice.
- Knowledge of community resources.
- Ability to function in high stress situations and manage multiple priorities while maintaining a positive, professional manner and outlook.
- Strong written and verbal communication skills.
- Demonstrated ability to assist callers in identifying their needs.
- Strong data entry skills and experience with electronic medical records (EMR)
- Ability to understand and follow somewhat complex verbal and written instructions in English.
- Communicates effectively in English both orally and in writing.
- Ability to continuously sit and work in front of a computer terminal and on the telephone.
- Ability to make frequent repetitive motions involving hands/wrists involved in writing, keyboarding and using the computer mouse.
Job Conditions:
- Office, home, hospital or other facility and automobile settings.
- The condition of patient home can vary including climbing stairs, poor ventilation, cluttered, unclean, and small confined space at times.
- Daily use of personal automobile. May experience poor driving conditions during inclement weather.
- Physical requirements: Patient lifting, repositioning, etc. Ability to lift 50 pounds unassisted.
