Licensed Practical Nurse (LPN) - Hospice

We are currently looking for a full-time LPN who wants to use their passion and expertise as part of an extraordinary nursing team to provide the best care possible for our patients. This position requires Tuesday- Saturday, 8 am to 5 pm availability, and will include some on-call assignments. Depending on your hospice and/or home health experience, our starting hourly rate range is $23.00 to $27.00 per hour.


Provide direct nursing care as indicated by the plan of care under the supervision of RNCM

Assess patient/family general physical and mental condition with the assigned RN Case Manager

Participate in the planning and coordination of patient care

Administers prescribed meds according to law as permitted within the scope of individual license

Provide patient/caregiver/family teaching and instruction as appropriate

Assist the patient with activities of daily living and encouraging appropriate self-care

Provide physical and emotional support to the patient/family in the terminal phases of life

Demonstrate respect and caring and maintains patient and staff confidentiality

Assists with provision of in-home respite and/or continuous care as patient need arises

Documents according to established agency guidelines and nursing practice standards


Current Oregon Licensed Practical Nurse license

Current CPR certification

Current Oregon driver’s license, dependable transportation, and proof of current automobile insurance

One year of hospice or home health nursing experience preferred

Knowledge of pain and symptom management.

Working knowledge of and the ability to apply the basic principles and techniques of nursing care to the hospice setting

Excellent communication skills and be able to utilize problem-solving teachings.

Ability to record and maintain patient data using an electronic record keeping system (EMR)

Ability to function in high stress situations and manage multiple priorities.

Ability to regularly walk bend, stoop, kneel, push, pull, grasp, reach, handle, and lift to 50lbs without assistance.
