The Role: 

As a Hospice Aide at Willamette Vital Health, you will play an important role in the life of a hospice patient and their family.

We’re looking for Hospice Aides that want to use their passion and expertise as part of a team to provide the best care possible. We have multiple openings with varying schedules (full time or part time)—find out if one works for you! Each position may have on-call rotation duties at the Edward F. Tokarski Home, our adult foster home for hospice patients. 

By providing expert personal care to hospice patients in their homes and in the Tokarski Home, you will bring dignity and compassion to patients in their final weeks and months of life.

Requirements:

  • High school diploma or GED
  • Current Oregon CNA License – may be an LPN
  • Minimum 1 year experience providing direct care to adults who are elderly or physically disabled in a hospice or home health care setting
  • Must be able to lift and carry up to 50 pounds, push up to 200 pounds in a wheelchair, and pull up to 100 pounds
  • Ability to frequently bend, twist, kneel, sit, stand, climb stairs, and make repetitive motions involving hands/wrists and feet involved in writing and driving
  • Must have reliable transportation, valid driver’s license, and proof of auto insurance in order to travel to and from patients’ homes
  • Must be fully vaccinated against COVID-19

Responsibilities:

  • Provide personal care services to patients under the supervision of the RN Case Manager
  • Assist in developing individualized care plans as part of the hospice team
  • Relate to patients and families in an open, empathetic, and supportive manner while maintaining personal and professional boundaries
  • Responsible for electronic charting for patients
  • Assist with light housekeeping duties as needed
  • Teach and empower family caregivers by providing information they need to care for their loved one confidently
  • When necessary, provide personal care services to Tokarski Home residents, assisting in the development of individualized care plans, medication management and charting for each resident, and assist with meal preparation and housekeeping
  • Identify safety issues and take appropriate actions
  • Recognize emergencies and institute emergency procedures; or notify appropriate people
