Nurse Practitioner

As a nurse practitioner, under the direction of the Hospice Medical Director, you will act as attending physician for select hospice patients, consult on hospice and selected palliative care patients, and support the hospice medical director and physicians.

The nurse practitioner will share in the on-call rotation with the hospice physicians. The nurse practitioner interfaces with other managers, hospice physicians, hospice interdisciplinary team members, and community physicians to ensure excellent patient care and customer satisfaction.

This is a full-time position, Monday thru Friday 8:00 am to 5:00 pm, with responsibilities including doctor of the day, face to face visits, and on-call duties.


  • Serves as a source of clinical expertise to hospice staff, agency, providers, and payer sources.
  • Acts as expert consultant or attending physician for hospice patients
  • Makes home visits to patients and families as needed
  • Consults on pain/symptom management issues
  • Evaluates patients to provide information for recertification discussions
  • Acts as resource for prescriptions when attending physician unavailable
  • Participates in interdisciplinary group discussions and meetings
  • Reviews patient charts and medications as requested and provides input on appropriate interventions or changes in the plan of care
  • May perform or assist in medical procedures at the patient’s residence, including intravenous or subcutaneous line placement, phlebotomy, and wound care
  • Maintains clinical knowledge to ensure the provision of excellent clinical care and to serve as resource to staff.


  • Demonstrated understanding of non-profit management, Medicare and CHAP standards, and quality management/review process.
  • Communicates effectively on an interpersonal and clinical level with physicians, nurses, and other healthcare staff.
  • Must have at least five years of nursing experience, part of which has been community based
  • Must be an adult, family, or geriatric nurse practitioner
  • Direct hospice or palliative care experience required
  • MSN from an accredited university or college
  • Education and/or experience to meet nurse practitioner requirements as defined by the Oregon State Board
  • Must possess a current Registered Nurse license in the State of Oregon and licensed as Nurse Practitioner by Oregon State Board of Nursing
  • Computer literacy including use of EMR required
  • Must have prescriptive authority
  • Must have a valid Oregon Driver’s license, proof of insurance and reliable transportation
