Tokarski Home Hospice Aide - Weekend Shift

When receiving hospice care in one’s own home is not an option, Willamette Vital Health offers the Edward F. Tokarski Home for our hospice patients who need a place to call home in their final months of life. The Tokarski Home is a five-bed adult foster care facility located in Northwest Salem, Oregon.

As a Hospice Aide at the Tokarski Home, you will provide personal care services for up to five (5) residents under the supervision of the RN Clinical Manager. You will assist in the development of individualized care plans, medication management, and charting for each resident, and you will assist with meal preparation and housekeeping.

The hours of work are Friday 7:00 am to 7:00pm, Saturday 7:00 am to 7:00 pm and Sunday 7:00 am to 7:00 pm. All three days are required = 36 hours

Essential Functions/Major Responsibilities:

  • Respecting the dignity, comfort, and privacy of each resident
  • Responsible to provide patient bathing, shampooing, skin and nail care, oral hygiene, toileting and elimination, normal range of motion and positioning, vital signs, safe transfer, and ambulation techniques, feeding and fluids, and use of adaptive devices
  • Housekeeping, including making beds, doing Tokarski patient laundry, and keeping the inside residence maintained.
  • Food preparation and cooking for residents.
  • Performs necessary housekeeping to provide quality environment for residents.
  • Assists with medication management and charts according to agency guidelines.
  • Assists with development of individualized care plans.
  • Identifies and communicates needs of residents and their families.
  • Reports on changes in resident conditions to a Registered Nurse.
  • Documents according to established guidelines and electronic medical records system.

Education/Experience/Requirements:

  • High school diploma or GED.
  • Certified as a Hospice Aide in Oregon - a current Oregon CNA license is required.
  • Must have an equivalent of three years of full-time experience providing direct care to adults who are older or adults with physical disabilities and who require full assistance in four or more activities of daily living.
  • Successful completion of Ensuring Quality Care (EQC) training and exam and all required Adult Foster Home Caregiver training.
  • Complete a physical capacities evaluation that indicates the ability to function under the specific job skills required for the position which are: Lift and carry up to 50 pounds, push up to 200 pounds in a wheelchair, and pull up to 100 pounds. Ability to frequently bend, twist and kneel. Ability to frequently sit, stand, and climb stairs.
  • Must have current Oregon driver's license, dependable transportation, and proof of current automobile insurance.
