Transitions is a free, non-medical program to help those facing a serious, life-limiting illness and their family. The Transitions Coordinator and trained volunteers will provide emotional support, assistance connecting with community resources, and help discussing and preparing advance directives and can assist in exploring future care needs.
Who is eligible?
Anyone with a serious, life-limiting illness.
Individuals that would benefit from assistance with finding community services to meet their needs and/or talking through their options and next steps.
How does it work?
The Transitions Coordinator or trained volunteer meets with the client and their families to determine their specific needs, puts the client and family in contact with appropriate existing community programs, is available to discuss and prepare advance directives and explore future care needs. The coordinator follows up with a client and their the family to identify further needs for about 90 days.
Who pays for the program?
Transitions is a free, volunteer-based program. Clients do not need to have health insurance. The program is sponsored by Willamette Valley Hospice and is funded through grants and donations.
