Patient Services Specialist – Part-Time

(Tuesday thru Sunday 8am to 5pm - hours vary)

As a Patient Services Specialist (PSS), you will provide specialized support for clinical staff and managers that facilitates our work with patients and families. You will regularly communicate with doctors, managers and staff to clarify, coordinate or obtain additional information needed for effective patient care. You will assure that all forms are completed, properly identified and signed and all required information is entered in the electronic medical record (EMR).

Duties

  • Generates, compiles, completes and forwards all required documentation needed for continuity of care for transfers.
  • Generates, tracks, and prepares required information for clinical meetings, distributing and posting agendas on the agency’s intranet, communicates with clinical managers and medical staff regarding any inaccuracies and/or anomalies.
  • Arranges and coordinates transportation for patients as requested by staff.
  • Prepares discharge of patients in the electronic medical record (EMR) system, and forwards required documents to physicians and medical facility according to policy and Medicare Rules.
  • Processes any patient deaths, notifying physicians and medical examiner as required according to state law.
  • Reviews charts for completeness and accuracy of key elements on discharge.
  • Communicates referral information to vendors, sends payment responsibility forms and physician orders as needed, and tracks to ensure results or notes are obtained afterward.
  • Monitors incoming and outgoing electronic faxes, forwarding to appropriate staff.
  • Prepares records for release, ensuring accuracy of signed release forms and documents are in place. Information released according to state and federal laws.
  • Communicates tasks needing completion for patient care after 5:00 pm and sends end of day report.

Requirements

  • Knowledgeable about and supportive of the hospice philosophy.
  • Ability to function in high stress situations and manage multiple priorities well.
  • Strong written and verbal communication skills.
  • Demonstrated ability to hold matters in confidence.
  • Detail oriented with excellent organizational skills.
  • Must relate in an open, empathetic and supportive manner while maintaining personal and professional boundaries.
  • Ability to establish and maintain effective working relationship with team members, clinical staff, patients and families, outside vendors, volunteers and the general public. Ability to work with multiple disciplines and individuals.
  • High School diploma or GED required.
  • Bi-Lingual English & Spanish preferred.
  • Health Information Management or Medical Office Management education preferred.
  • Formal training and/or work experience in a health care setting preferred.
  • Knowledge of basic HIPAA rules and medical terminology required.
  • Two years experience in various aspects of medical office practices required.
  • Computer literacy, typing, filing, multi-line telephone, data entry, report processing and facsimile transmission skills required.
  • Must have current Oregon driver’s license, dependable transportation, and proof of current automobile insurance.
  • Ability to work flexible hours including weekends and early evenings.
