Admissions Coordinator - LPN

This is an hourly, non-exempt, full-time position with work hours Monday-Friday 7:30 am – 4:30 pm.

As an admissions coordinator, you will act as a bridge between referral sources, patients and their families, and Willamette Valley Hospice staff. You will help families that are experiencing the difficulty in realizing their loved one is nearing the end of their life. You will be a positive, welcoming, empathetic representative of the WVH team to healthcare providers, patients, and families. We are looking for an LPN that wants to use their passion and expertise as part of a team to provide the best care possible for hospice patients.

Duties

  • Manage referrals and all components associated with the process, including coordination with the eligibility RN, scheduling program explanations, transportation, and DME.
  • Communicate with physicians to obtain admission related physician orders and medications
  • Enhances patient admissions process by requesting and documenting additional referral information to facilitate timely assessments
  • Use your teaching skills to empower patients and family caregivers as you explain hospice services and WVH programs to help them identify their options for care to optimize quality of life
  • Provide timely documentation and communication

Requirements

  • Current Oregon LPN license
  • Ability to work in a fast-paced environment with multiple priorities and periods of time requiring a high volume of work
  • Demonstrated teamwork through cooperation, flexibility, communication, dependability and supporting team goals
  • Knowledge of hospice concept and practice required
  • Previous hospice experience preferred
  • Customer service experience preferred
  • Computer literacy and experience with electronic medical record keeping systems
  • Current Oregon driver’s license, dependable transportation, and proof of current automobile insurance
